Required after every Intensive Service shift — filed in the client’s official service record
Complete and submit after every Intensive Service shift.
Your note becomes part of the client's official service recipient record as a daily log / progress note (
Minn. Stat. §245D.095, subd. 3(b)(11)
) and is how Bella Mente documents progress toward the outcomes in the client's support plan addendum (
Minn. Stat. §245D.071, subds. 4–5
). Notes may be reviewed by DHS licensors, case managers, and the client or their legal representative — write accordingly.

3. Progress Notes
What a complete note contains
(SMART documentation):

pecific — name the support plan goal worked on and what you and the client actually did.

M
easurable — record observable progress, data, or milestones (245D requires measurable, observable criteria — §245D.071, subd. 4(b)(2)).

A
chievable — note supports or methods used and how the client responded.
•
R
elevant — tie the activity to the client's support plan outcomes and priorities.
•
T
ime-bound — reference timelines, target dates, or progress markers.
Do:
write in first person, objective and factual; include the client's participation and response; note barriers and the plan for the next visit.
Don't:
include other clients' names or identifying details, opinions about family members, or diagnoses you have not been told.
